PARENTAL CONSENT FORM

For events explicitly marked 16+, guests aged 16 to under 18 are admitted until midnight at the latest if they bring this consent form, filled out completely, in the original.

The form only counts as an original, not as a copy, photo or digital version. It requires a copy of the parent's ID and the named accompanying adult on site. The full rule is in the house rules.
