PARENTAL CONSENT FORM

For events explicitly marked 16+, guests aged 16 to under 18 are admitted if they bring this consent form, filled out completely, in the original. Without the accompanying person named in it, the stay ends at midnight at the latest; if that person stays present throughout, it can run longer.

The form only counts as an original, not as a copy, photo or digital version. It requires a copy of the parent's ID and the named accompanying adult on site. The full rule is in the house rules.
